Job Description
Our client, a leading provider of community health services, is looking for a dedicated and compassionate Registered Nurse to join their team in Port Harcourt. This role focuses on delivering essential healthcare services to individuals and families within the community, promoting health education, and managing chronic conditions. The position requires a blend of on-site patient care and potentially some remote administrative or follow-up tasks. You will play a vital role in improving health outcomes and ensuring access to care for underserved populations.
Responsibilities:
Provide direct nursing care to patients in their homes or community settings. Conduct health assessments, develop care plans, and administer treatments as prescribed. Educate patients and families on disease management, medication adherence, and healthy lifestyle choices. Monitor patient progress and report changes to physicians or healthcare providers. Administer immunizations and perform basic diagnostic tests. Manage chronic diseases and provide support for patients with long-term health conditions. Maintain accurate and confidential patient records. Collaborate with social workers, therapists, and other healthcare professionals. Respond to emergency situations and provide first aid as needed. Participate in community health initiatives and outreach programs.
Qualifications:
Valid RN license in Nigeria. Minimum of 3 years of experience as a Registered Nurse, preferably with experience in community health, public health, or home healthcare. Strong clinical skills and knowledge of current nursing practices. Excellent assessment, critical thinking, and problem-solving abilities. Compassionate and patient-centered approach to care. Effective communication and interpersonal skills for interacting with patients, families, and colleagues. Proficiency in electronic health record (EHR) systems is a plus. Ability to work independently and manage time effectively. Must possess a valid driver's license and have access to reliable transportation. Commitment to maintaining patient confidentiality and ethical standards.
