Job Description
Our client is seeking a compassionate and experienced Pediatric Home Care Nurse to provide remote clinical support and patient coordination. This hybrid role involves leveraging technology to connect with patients and families, offer guidance, and manage care plans, while also requiring occasional on-site visits or in-person team meetings. You will play a vital role in ensuring continuity of care, educating families, and facilitating seamless transitions for pediatric patients requiring home-based medical services. The ideal candidate possesses strong clinical skills, excellent communication abilities, and a passion for pediatric healthcare. Experience with telehealth platforms and electronic health records is essential.

Key Responsibilities:
Provide remote clinical guidance and support to pediatric patients and their families. Conduct virtual patient assessments and monitor health status via telehealth platforms. Develop, implement, and evaluate individualized care plans in collaboration with physicians and other healthcare professionals. Educate families on disease management, medication administration, and home care techniques. Coordinate patient care services, ensuring timely access to necessary resources and appointments. Serve as a primary point of contact for families, addressing concerns and providing emotional support. Facilitate communication between families, healthcare providers, and community resources. Document all patient interactions, assessments, and care plans accurately and timely in the EHR system. Participate in regular virtual team meetings and case conferences. Identify potential risks and implement preventative measures to ensure patient safety. Assist with medication reconciliation and adherence monitoring. Provide training and support to other remote care team members as needed. Stay updated on best practices in pediatric home care and telehealth. Conduct occasional in-person visits for assessments, training, or critical care coordination as required by the hybrid model. Advocate for the needs of pediatric patients and their families within the healthcare system.
Qualifications:
Registered Nurse (RN) license in Nigeria. Bachelor of Science in Nursing (BSN) or equivalent. Minimum of 5 years of experience in pediatric nursing, with at least 2 years in home healthcare or a related field. Experience with telehealth platforms and remote patient monitoring technologies. Proficiency in electronic health record (EHR) systems. Strong understanding of pediatric developmental stages and common childhood illnesses. Excellent assessment, critical thinking, and problem-solving skills. Exceptional interpersonal, communication, and patient education skills. Ability to work independently and manage time effectively in a hybrid work environment. Empathy, compassion, and a patient-centered approach. Flexibility to travel for occasional required in-person interactions within the vicinity of Jos, Plateau, NG . Tech-savviness and comfort with virtual collaboration tools.
